As mentioned in the comments (was the first thought that popped into my head as well), 3 of the 4 vehicles belong to the same fleet company.  Fleet mechanic overtorquing lugs maybe.

"Fracturing," low miles (not anywhere near a tire change interval), and relegated to a single customer, to me says Ford probably got a bad batch of studs from a supplier.

Over torquing with an impact wrench will immediately break the studs - there ain't no half-way stressing there. Those babies will come right off.

I can see bad studs being possible.  When I sold parts every one of them we carried were made in China.
