My boss in high school forgot to put the ones back on the right side of his MG before going down the road. He made it a long way down the road before they came off. We helped him by rolling on the ground laughing.

The lugs were loos on one of the wheels of the MG when I got it... Crazy old man wasn't strong enough to tighten them all the way.

IIRC the story is that he had just bought the wheels/tires at a tire shop and they gave him a set of lug nuts that didn't quite match up with the wheels, causing lack of good clamping force, or something like that.
But all four at once?
Yeah I dunno. Could be that a lack of thorough tech inspection is the problem too. I'd imagine that if you took a torque wrench to those babies before the run it wouldn't have been a problem.
I don't know this happens. I've accidentally driven a short distance with loose lugnuts (lower the car to the ground to do the final tightening and forget), and it's pretty obvious that something is weird.

When they shipped my minivan from Germany to the US it somehow got a flat.
I'm glad the wife wasn't driving the van from the port, she wouldn't have noticed it the wobbly wheel until it fell off!
